Immigration physicals are available at our 1309 N. East Street location with Dr. Larry Balzer. The guidelines for physical are as follows and through appointment only:
- PCHC new patient packet must be completed
- Vaccine record must be provided
- Valid/Non-Expired picture ID must be provided
- Legal name changes must be proven with government issued documents
- Interpreter who can read, write, and speak English must be provided by patient
- Patient must have COVID-19 vaccine prior to physical appointment.
